Appointments

Dr. Chavis sees patients in Indianapolis and Kokomo. By calling 317-299-7603, his staff can schedule an appointment at the location most convenient for you. Personalized attention is our goal and each patient is scheduled into clinic as soon as possible. If it is necessary for you to cancel an appointment, we would appreciate 24 hours advanced notice. Emergency patients can be seen immediately either in the office or emergency room depending upon the condition.

During your clinic visit, we recommend that you ask any questions and voice any concerns that you may have about your procedure, the possible complications and any available alternatives. Because a well-informed patient is often the happiest, this open line of communication between Dr. Chavis and his patients is the most effective way to maintain understanding and achieve a satisfactory outcome.

Payments & Insurance
Payment for office visits and procedures is required at the time of treatment. Cosmetic surgery is rarely, if ever, covered by private insurance. Because of that, we are affiliated with a financing institution and also accept most major credit cards.

Dr. Chavis also participates with Workers' Compensation for treatment of acute and chronic upper extremity injuries from the elbow to the fingertips.
